And 15% off on restorative fillings
The list below is an agreement with each of the following:
- A yearly recurring fee of $280 to begin saving the plan.
- Full payment renders and due at the time of services.
- Terms and fees may change at the time of the services.
- The plan DOES NOT cover Implants, Veneers, or Whitening treatments.
