Office savings plan annual enrollment gives you the following:

  1. Exam and x-rays (D0150, D0210, D0330)
  2. Prophy (D1110)
  3. Emergency visit (D0140, D0220)

And 15% off on restorative fillings

The list below is an agreement with each of the following:

  • A yearly recurring fee of $280 to begin saving the plan.
  • Full payment renders and due at the time of services.
  • Terms and fees may change at the time of the services.
  • The plan DOES NOT cover Implants, Veneers, or Whitening treatments.

This plan is a savings plan for retired veterans and patients with no dental insurance and cannot be combined with any other offers; THIS IS NOT A DENTAL INSURANCE.
